I am the person processing this transaction and am 18 years of age or older and the registered member/participant, or I am the person processing this transaction and am the legal parent/guardian of the member(s)/participants(s).

I authorize the YMCA of Metropolitan Los Angeles to charge the credit card or bank account indicated for the noted amount(s) on the date(s) indicated. This payment is for membership or program dues payments. I certify I am an authorized user of this credit card or bank account and will not dispute the payment with my bank or credit card company, so long as the transaction corresponds to the terms provided.
